Transforming Palliative Care Delivery in Halton

Acclaim Health, in partnership with Connected Care Halton OHT and Burlington OHT, offers fully funded Palliative Care Clinical Coaching for participating healthcare organizations across our region as part of a provincial initiative to strengthen access to high-quality palliative care.

Coaches work directly with staff and leadership teams in Community and Hospital settings to drive practice change through coaching, education and strengthening access to high quality palliative care.

They support practice changes to help guide organizations and leaders in the implementation of the provincial Palliative Care Health System Delivery Framework (HSDF) comprised of three Models of Care, supported by the Ontario Palliative Care Competency Framework, and tools to support earlier identification for palliative care.

How We Support Your Organization

Our clinical coaches help providers and organizations recognize the important role they play in identifying and supporting palliative care needs early for individuals living with serious illness.

We partner with your team to strengthen existing processes, workflows, and community connections by:

  • Facilitating access to provincial education opportunities.
  • Assisting with the practical application and integration of knowledge and tools through reinforced coaching and mentorship.

How It Works

Initial Engagement & Assessment
We begin with introductions and general discussions regarding your role, focus areas, and alignment. Together, we evaluate your organization’s current state and existing processes to identify strengths and key areas of opportunity.

Tailored Support Plan
Based on our assessment recommendations, we collaborate with your team to identify education needs and develop targeted quality improvement and practice change initiatives.

Implementation, Review & Sustainability
Coaches actively support the rollout of your initiatives through ongoing coaching, education, and mentorship. We continuously evaluate progress, adapting processes as needed to ensure long-term sustainability and alignment with provincial quality standards.
