Consultation and Mentorship
Case-based palliative care consultations available in person, by phone or via email for care providers in Halton and Peel.
We assist community front line providers in:
- Long Term Care Homes
- Ontario Health at Home and contracted service providers (community nursing and community support services)
- Hospices
- Community programs such as day programs, supportive housing, etc.
Consultation
Case-based palliative care nurse consultations are available in person, video-conference, phone or by email (if email – no personal health information). Consultation is an active dialogue between the PPSMC and the individual’s health care team to support case-based education/coaching (they do not provide direct patient care or services to the general public). The provider/health care team requesting support must be available to participate. Consultations are free and a doctor’s referral is not required.
Our team is available Monday – Friday from 8:30am – 4:30pm; we are not an emergency or after-hours service.
Consultation for palliative care program development, including:
- Support with application of assessment tools and best practices
- Mentorship for palliative/pain leads or champions in organizations
- Palliative care committee and pain/comfort care rounds program support
- Linking service providers with evidence informed tools, resources, best practice guidelines and specialized palliative care resources
- Collaboration to support local implementation and practice changes in partnership with other programs, projects or resources
