Home Nursing

Nursing is where it all started for us.

We have been providing home nursing for over 85 years, and we are proud to carry on this tradition of exemplary care and service in our community.

As always, our caring, friendly and knowledgeable nurses are known for the exceptionally high quality care they provide.

You may require nursing care if you:

  • Have a chronic health condition
  • Are recovering from surgery, an illness or injury
  • Are facing a life-limiting illness
  • Require an IV treatment

Our specialty nurses are second to none and are a lifeline for people experiencing life-changing health issues.  Our team includes experts in:

  • Wound Care (see video below)
  • Continence Management
  • Enterostomal Therapy
  • Palliative Care

Current Home Nursing Patients

Our Customer Service team is available from 7:00am-11:00pm, seven days a week, at 905-827-8800 or 1-800-387-7127.

Please contact our team if you need to change or cancel your appointment, have questions about your care, or experience a sudden change in your condition (in an emergency, call 911).

Overnight, between 11:00pm and 7:00am, urgent calls are monitored by an on-call Supervisor.

Government-Funded Home Nursing

If you qualify, home nursing can be provided through the health care system at no cost to you.  Acclaim Health is one of the providers in Halton that are able to provide government-funded nursing.

To find out if you qualify for this free support, you must be assessed by the Local Health Integration Network (LHIN) in your area.  You can call them to discuss your needs or have your family doctor complete a referral.

In Oakville, Milton and Halton Hills contact:
Mississauga Halton Local Health Integration Network (MH LHIN) Home and Community Care at 1-877-336-9090.

In Burlington contact:
Hamilton Niagara Haldimand Brant Local Health Integration Network (HNHB LHIN) Home and Community Care at 1-800-810-0000.

 

Private Home Nursing

Additional nursing care is available from Acclaim Health.  We can help for one hour up to overnight and over several days.  Pricing is dependent on the duration of care required.
